Initial Psychiatric Evaluation (75 minutes)
$450
A comprehensive psychiatric evaluation that includes diagnostic assessment, treatment recommendations, medication review, and an individualized care plan.
45-Minute Follow-Up Appointment
$275
Ideal for medication management combined with supportive therapy, lifestyle interventions, and detailed treatment planning.
20-Minute Follow-Up Appointment
$150
Designed for established patients requiring focused medication management and symptom follow-up.
Insurance
Insurance credentialing is currently in progress. Until then, Kind Mind & Wellness is accepting self-pay patients and can provide a superbill for those seeking possible out-of-network reimbursement.
POLICIES
Payment & Policy
A valid credit or debit card is required to reserve your appointment. Payment information will be securely stored on file and will be charged at the time of your scheduled appointment.
Cancellation Policy
We kindly ask for at least 48 hours’ notice if you need to cancel or reschedule your appointment.
Appointments canceled with less than 48 hours’ notice will incur a fee equal to 50% of the scheduled appointment cost.
No-show appointments (failure to attend a scheduled appointment without prior notice) will be charged 100% of the scheduled appointment fee.
These fees will be charged to the payment method on file.

ADDITIONAL FEES & PRACTICE POLICIES
Ketamine Treatment
Ketamine treatment is through a specialized compounding pharmacy. The cost of the medication and pharmacy services is separate from psychiatric appointment fees.
Professional Services
Professional services outside of scheduled appointments—including, but not limited to, completing forms, disability or accommodation paperwork, reviewing extensive records, or writing requested letters—are billed at a prorated rate of $400 per hour.
Stimulant Prescription Refills
For patients prescribed stimulant medications, refill requests made outside of a scheduled appointment are $25 per prescription. This fee is waived for Signature Care members. Legal & Forensic Services
